Description

Gynecological cancers—those affecting the female reproductive system—are a major cause of cancer-related deaths among women in India. Among them, cervical cancer remains the most common, largely due to infection with the Human Papillomavirus (HPV). In Rajasthan, increasing awareness, improved screening programs, and access to specialized cancer care are helping detect these cancers earlier and improve survival rates.

Types of Gynecological Cancers

The main types include cervical, ovarian, uterine (endometrial), vaginal, vulvar, and fallopian tube cancers. Cervical cancer is the most prevalent in India, while ovarian cancer is often diagnosed at a later stage due to subtle symptoms. Uterine cancer cases are rising due to lifestyle factors such as obesity, whereas vaginal and fallopian tube cancers are relatively rare but treatable when detected early.

Risk Factors

Several risk factors contribute to the development of gynecological cancers. HPV infection is responsible for over 95% of cervical cancer cases. Other key risks include early sexual activity, multiple partners, lack of HPV vaccination, obesity, sedentary lifestyle, and hormonal imbalances such as those seen in PCOS. A family history of breast or ovarian cancer and BRCA gene mutations also significantly increase the risk, especially for ovarian cancer. Most gynecological cancers are more common in women over the age of 40.

Symptoms to Watch For

Early detection plays a critical role in successful treatment. Warning signs include abnormal vaginal bleeding (especially after menopause or intercourse), unusual discharge, pelvic pain, abdominal bloating, and a feeling of fullness. Some women may also experience pain during intercourse, urinary issues, unexplained weight loss, or fatigue. Unfortunately, social stigma and lack of awareness often delay medical consultation, especially in rural areas.

Diagnosis and Screening

Routine screening and timely diagnosis can save lives. A Pap smear is the most effective screening test for cervical cancer and is recommended for women aged 21–65. HPV DNA testing helps identify high-risk infections. Additional diagnostic tools include colposcopy, transvaginal ultrasound, MRI, PET-CT scans, and tumor markers like CA-125. For uterine cancer, an endometrial biopsy is commonly performed, while BRCA testing is advised for those with a strong family history.

Treatment Options

Treatment depends on the type and stage of cancer. Surgery is often the primary approach and may include procedures such as hysterectomy or tumor removal. Advanced techniques like laparoscopic and robotic-assisted surgeries allow faster recovery. For ovarian cancer, debulking surgery and HIPEC (heated chemotherapy delivered during surgery) are increasingly used in advanced cases.

Non-surgical treatments also play a vital role. Radiation therapy, especially brachytherapy, is highly effective for cervical cancer. Chemotherapy—often platinum-based—is commonly used for ovarian cancer, sometimes combined with targeted therapy drugs like PARP inhibitors. Immunotherapy is another promising option for advanced cancers that do not respond to traditional treatments.

Prevention and Awareness

HPV vaccination is one of the most powerful tools for preventing cervical cancer. It is recommended for girls aged 9–14 and can significantly reduce future cancer risk. Expanding vaccination programs and awareness campaigns across Rajasthan, particularly in rural communities, is essential.

Conclusion

Gynecological cancers can be effectively managed if detected early. Awareness, regular screening, and access to expert care are key to improving outcomes. Women should never ignore persistent symptoms and should seek timely medical advice. With the right care and preventive measures, the burden of these cancers in Rajasthan can be significantly reduced.
